BLAZE (EVANS) - Amy Helen Evans Blaze, age 32, passed away April 5, 2009. She will be deeply missed by her daughter, Edie; mother, Nancy Evans; father, Bruce Evans; brother and sister-in-law, Ben and Kristen Evans; seven aunts and uncles, many cousins.

A memorial service will be held Monday, 1 p.m. at Kentwood Community Church, 1200 60th SE, Grand Rapids, MI 49508. Relatives and friends may meet her family Monday from 12 noon until the time of the memorial service. Her family welcomes memories and messages in their guest book online at www.cookfuneralservices.com.

Contributions in memory of Amy may be made to the Edie Blaze Trust Fund at any Fifth Third Bank.
